What to Bring to Your First Visit

You are not sure what to wear, whether the x-rays from last year matter, or how much of this you are supposed to organize beforehand. Short version: comfortable clothes you can move in, whatever imaging and notes you already have, and yourself, rested. There is nothing to print and nothing to download. Your intake forms are filled out here at the clinic, and no referral is needed, because Texas is a direct access state.

01What to Bring

A short list, and none of it involves a printer.

Your first visit is 45 to 60 minutes, one on one with a Doctor of Physical Therapy. Most of that time goes to finding out why the thing hurts and starting to do something about it, so the preparation on your end is deliberately light. Come able to move, bring whatever history you already have on hand, and let us handle the rest. If you are not sure whether something is worth bringing, bring it.

Wear something loose enough that you can bend, squat, reach overhead and lie down without fighting your own waistband. Avoid anything that restricts circulation. We need to see the joint that is bothering you actually move, so shorts help for a knee, hip or ankle, and a sleeveless top or tee helps for a shoulder or neck. Nothing you would not wear to a gym.

Depending on your condition, you may be advised to bring particular footwear or a brace. Running shoes matter if we are looking at gait or a return to sport. Work boots matter if the pain shows up on shift. A brace, sleeve or orthotic you already own tells us what you have been relying on and whether it is helping. If nobody has asked you for anything specific, regular sneakers are fine.

Bring what you already have: x-rays, ultrasounds, doctor's notes, surgical reports, a list of your current medications. You do not need to chase anything down before you come, and you do not need a referral to start. What you bring simply gives Dr. Egbuzie more to work with, and it saves you repeating a story you have already told several people.

Arrive rested if you can manage it. This is not a session you sit through. You will be moving, tested, treated, and taught what to do between visits, and the parts you remember are the parts that carry the recovery forward. Turning up straight off a double shift with no sleep gets you a less useful hour than turning up ready to work.

Keep track of what you took and when. Pain medication changes what you feel during treatment, which changes how hard we should push and where the honest limits are. It is not a problem that you took something. It is only a problem if we do not know, because then the intensity gets set against a signal that is temporarily turned down.

02Your Forms

The intake happens here, not at your kitchen table.

There is nothing on this page to download, because there is nothing you need to print. Intake forms are completed at the clinic when you arrive, and they are short. We would rather ask you the questions in person, where an answer can turn into a follow up question, than hand you a stack of boxes to tick at home and hope the important detail makes it onto the page.

01

You arrive

Come to 7340 S State Hwy 78 Unit 300, Sachse, TX 75048 (Inside Fitness Xpress). Give yourself a few minutes at the start for the forms. If anything about parking or the entrance is unclear, call and we will talk you in.

02

You fill out the intake here

Contact details, medical history, what hurts, and what you want to get back to doing. It is short, and Dr. Egbuzie reads it before she sees you rather than after.

03

The evaluation starts

The rest of your 45 to 60 minutes is the actual appointment: history, hands on assessment, testing how you move, and a first round of treatment. You leave knowing what is wrong, what the plan is, and what to do before the next visit.
